Terms and Conditions Your will get the following document to sign once you have purchased one of our programmes. This outlines the framework for services at ADB Nutrition, provided by Anastasija den Blanken or Rebecca Traylen, from herein referred to as consultant. Your consultant will go through it with you in the first session of your work together. Please talk with them if there is anything you would like clarified and sign the bottom of this form to acknowledge your understanding and agreement to these terms.Consultant practices in accordance with scope of practice from the below: the Federation of Nutritional Therapist Practitioners and UK & International Health Coaches Association as Full Member (Anastasia). Association for Nutrition as Registered Associate Nutritionist (Rebecca) Consultations You will discuss and agree what support you want from your consultant in your first session and review the progress and focus of your work together as you go along. Confidentiality Information about you disclosed during your sessions or in the pre-consultation forms will be kept confidential. If your consultant has serious concerns about your safety, or the safety of another person, normal confidentiality may be lifted. You will be asked to provide your GP’s contact details, as someone they may contact in these circumstances. Consultant keeps a record of your name, contact details and dates and times of your appointments on a password-protected database. Consultant is required to keep notes about the focus of your work. These are kept securely and are password protected. Any reports produced by Consultant to evaluate the service contain anonymised data and will not contain any identifying information of clients. Working with Multiple Therapists Consultant may suggest you see other registered therapists to support your goals. Information about your symptoms and condition will be discussed between Consultant and other therapists so that we can provide you with a co-ordinated therapy plan. Consultant will ask for your informed consent to share information with the other registered therapists and ask if there is anything that you want to remain confidential. Consultant will help you assimilate any information gathered from other registered therapists. Working on Phone & Google Meet It is your responsibility to have a working internet connection for all Google meet sessions. You will be charged for the scheduled sessions if Consultant is waiting for your call, but you are unable to connect online. Consultant will call you for any phone sessions to a UK mobile number. WhatsApp calls are possible as well. Payments Full payment is required at the time of booking, prior to your first appointment. This is made via invoicing system (QuickBooks). You can pay via Paypal using your credit or debit card, or make a bank transfer. Cheques are not accepted. More details will be or have already been provided to you in the Welcome letter. Timeframes The 1-month (introduction) package must be completed within 3 months, upon booking.The 3-month package must be completed within 6 months, upon booking.The 6-month package must be completed within 10 months, upon booking.Membership plans have to be used once purchased as you will be billed every month . Allow 5-7 working days from your first session to receive your personalised plan and handouts. More details will be or have already been provided to you in the Welcome letter. Cancellations The consultant appreciates that circumstances can change. If you need to rearrange an appointment, please let us know at your earliest convenience. Should you choose to cancel your programme , for any reason, at any point within 48 hours of your first appointment or afterwards , no refunds will be given . Cancelling your programme up to 48 hours before your first appointment will trigger a full refund. This is for package plans. You can cancel your membership plan at any point, but no refund will be given if you have already started your programme and had sessions with your nutritionist, therefore received and benefited from the service. Please note that you WILL be charged for missed session administrative fee (£30) if you did not email or call to let your consultant know that you will not be joining the session. No shows might also be charged administrative fee. We respect your time, so expect you to respect ours too. More details will be or have already been provided to you in the Welcome letter. Clients under 18 years of age I welcome consultations with clients under 18, but they must be accompanied by a parent or legal guardian. Supplements Largely supplements are recommended from Healthpath Pro, which is a marketplace for a range of products of the utmost quality. A commission agreement is in place between the company and the practitioner and a percentage of this is passed on to clients as a discount. Some supplements may be available cheaper elsewhere and you are welcome to source the exact product recommended from another supplier. Complaints If you are having a problem with your consultant, or the service, which you have not been able to resolve with your consultant, you may raise this by emailing contact@anastasiadenbalnken.com who will support you in finding a resolution. The Nutritional Therapy Descriptor This part will differ, depending on your consultant Nutritional Therapy (the service) is the application of nutrition science in promoting health, peak performance and individual care. Nutritional therapy practitioners use a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address nutritional balance and help support the body towards maintaining health. Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing . Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a ‘one size fits all’ approach. Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with ‘red flag’ signs or symptoms to their medical professional. They will also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client’s care to explain any nutritional therapy programme that has been provided. The Nutritional Therapist (NT) requests that the Client notes the following: The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme. Nutritional advice will be tailored to support health conditions and/or health concerns identified and agreed between both parties. Nutritional therapists are not permitted to diagnose or claim to treat medical conditions. Nutritional advice is not a substitute for professional medical advice and/or treatment. Your Nutritional Therapist may recommend food supplements and/or functional testing as part of your Nutritional Therapy programme and may receive a commission on these products or services. Standards of professional practice in Nutritional Therapy are governed by the National Occupational Standards (more here: https://www.fntp.org.uk/wp-content/uploads/2016/02/Nutrition-cnh8.pdf). This document only covers the practice of Nutritional Therapy within this consultation, and your practitioner will make it clear if he or she intends to step outside this boundary.The serviceYou will receive from your consultant: Personalised nutritional advice and meal plan suggestions : verbal during the session, and written (protocol and/or brochures and information leaflets). You might receive links to Youtube videos, articles and other interactive forms of learning more about health topics. Personalised exercise suggestions: this can be in verbal form and/or written within your protocol. You might receive links to Youtube videos, articles and other interactive forms of learning more about health topics. Personalised lifestyle suggestions: this can be in verbal form and/or written within your protocol or sent as a brochure/information leaflet. Areas we cover are sleep management, stress management, work/life balance and more. You might receive links to Youtube videos, articles and other interactive forms of learning more about health topics. Coaching: as a qualified health coach your consultant will support your journey through behavior coaching and motivational interviewing. This will be provided verbally during your sessions and/or via WhatsApp chat. You might receive some coaching activities to do on your own time after the session. Please ask your consultant about times when she can be contacted. Supplements advice: you will receive personalised supplement advice. This will be provided in a protocol form via the client portal. Functional testing: you will receive personalised functional testing suggestions. This will be provided in a protocol form via the client portal. You will receive an interpretation of the results from your tests. Please note that these interpretations are not to diagnose medical conditions. Personalised plan, or protocol: this includes your nutritional advice, lifestyle suggestions, supplement plan and functional testing suggestions. This is sent to you via the client portal. You may get additional documents via Google Drive. Please note that protocol is not a treatment plan for your medical condition. Accountability and support: as a qualified health coach your consultant will support your journey through behavior coaching and motivational interviewing. This will be provided verbally during your sessions and/or via WhatsApp chat. You might receive some coaching activities to do on your own time after the session. Please ask your consultant about times when she can be contacted. The Client understands and agrees to the following: I am responsible for contacting my GP about any health concerns. I give permission for you to contact my GP regarding any agreed aspects of my case. I understand that my consultant might consult with another healthcare professional about my case if needed: a dietitian, nutritional therapist, GP consultant and/or other. If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional strategy provided by my nutritional therapist. This is necessary because of any possible reaction between medication and the nutritional programme. It is important that I tell my nutritional therapist about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme. If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my nutritional therapist promptly for clarification. I must contact my nutritional therapist should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions. Recording consultations using any form of electronic media is not allowed without the written permission of both my Nutritional Therapist and me. In accordance with the new GDPR regulations in 2018, my personal data will be securely stored within my clinical practice for 8 years after which they will be securely destroyed. You are entitled to request a copy of your data and ask to destroy your profile at any time within 8 years from the day of your last consultation.
